Home-based skilled nursing care designed to improve the quality of life and function for someone who has difficulty leaving their home due to an injury, surgery, illness or chronic condition.
Palliative care focuses on Home Health patients who are not ready for hospice. This service may be adequate for people who are still receiving chemotherapy, dialysis and other terminal care.
Hospice is a service provided to a patient to assist with care when they have an illness that could result in death six months or less if the disease follows its course. Many conditions can lead to death; this may include but not limited to cancer, congestive heart failure, chronic obstructive pulmonary disease, end-stage dementia/Alzheimer's disease, and many others.
