Our responsibilities
Home Health & Hospice of Kansas is required by law to maintain the privacy and security of protected health information, provide this notice of our legal duties and privacy practices, follow the notice currently in effect, and notify affected individuals following a breach of unsecured protected health information when required by law.
How we may use and disclose health information
We may use and disclose health information for treatment, payment and healthcare operations. This may include coordinating care with physicians and other providers, billing health plans, reviewing quality, training staff and conducting healthcare business activities.
We may also use or disclose information when permitted or required by law, including for public-health activities, health oversight, reporting abuse or neglect, workers’ compensation, judicial or administrative proceedings, law enforcement, organ donation, coroners and medical examiners, disaster relief, national security and preventing a serious threat to health or safety.
Other uses and disclosures require written authorization when required by law. You may revoke an authorization in writing, except to the extent we have already acted in reliance on it.
Your rights
- Inspect or obtain a copy of your health information, subject to legal limitations.
- Request a correction to health information you believe is incorrect or incomplete.
- Request confidential communications at a different address or by another reasonable method.
- Request restrictions on certain uses or disclosures. We are not required to agree to every request.
- Request an accounting of certain disclosures.
- Receive a paper copy of this notice.
- Choose someone to act on your behalf when legally authorized.
- File a complaint without retaliation.
Your choices
You may tell us your preferences regarding sharing information with family, friends or others involved in your care, disaster-relief efforts, fundraising communications and other situations where the law gives you a choice.
